New Customers
If this is the first time your company has ordered from us, payment for your first order, or until you are approved for credit, is by C.O.D. or Credit Card. Once you complete a Credit Application (pdf download) and we approve you for credit, all future orders will be billed by invoice.

Returning Customers
Send us the casting of your patient along with a completed AFO Order Form (pdf download).

AFO Order Form
Please include a completed AFO Order Form (pdf download) for each casting you send us.

AFO Casting Instructions
Because our AFO's are custom made, we rely on you, the practitioner, to send us an accurate casting of your patient. Please use our Casting Instructions as a guide to help prevent any delay in your order.
